ABSTRACT

El síndrome de Sjögren (SS) es una enfermedad autoinmune que afecta a las glándulas salivales y lagrimales. Se expone el caso clínico de una paciente de 67 años de género femenino que consulta por dolor en múltiples dientes; relata sensación de boca seca (xerostomía) y sequedad ocular (xeroftalmia). Al examen intraoral, se observan múltiples caries en superficies atípicas, mucosas secas, saliva espumosa y notoria depapilación lingual. Se sospecha de SS, derivando a medicina interna y confirmándose el diagnóstico. En paralelo, se inicia el tratamiento odontológico, realizando adaptaciones en los procedimientos para aliviar la sintomatología del SS, especialmente durante tratamientos endodónticos.


Sjögren's syndrome (SS) is an autoimmune disease that affects the salivary and lacrimal glands. The clinical case of a 67-year-old female patient who consulted for pain in multiple teeth is exposed. Additionally, she reports a sensation of dry mouth (xerostomia) and dry eyes (xerophthalmia). During the intraoral examination, the following findings are noticed: multiple cavities on atypical surfaces, dry mucous membranes, foamy saliva, and atrophic glossitis. SS is suspected, referring to internal medicine and confirming the diagnosis. In parallel, dental treatment is initiated, making adaptations in the procedures to alleviate the symptoms of SS, especially during endodontic treatments.

Introdução: O desenvolvimento de novas técnicas, tecnologias e materiais influenciam a prática clínica da endodontia e impactam consideravelmente a forma como o tratamento do canal radicular é realizado. Embora existam informações sobre as várias formas de tratamentos praticados por dentistas em outras cidades e/ou países, muito pouco se sabe sobre as endodontias realizadas em clínicas odontológicas na cidade de Porto Alegre/RS. Objetivo: Analisar a prática endodôntica realizada por dentistas nas clínicas odontológicas na cidade de Porto Alegre/RS, visando explorar as técnicas, materiais e equipamentos empregados durante o tratamento. Material e método: Foi aplicado um questionário online, por meio da Plataforma Google Forms, com vinte e seis questões abertas e fechadas aos dentistas que realizam tratamentos endodônticos em clínicas odontológicas. O questionário foi estruturado a fim de analisar os materiais, técnicas e equipamentos que estão presentes nos tratamentos endodônticos realizados pelos dentistas nas clínicas odontológicas aqui avaliadas. Os dados coletados nos questionários foram transferidos para uma planilha Excel®. A análise dos dados foi feita de duas formas: frequência absoluta e relativa para as questões fechadas, e descritiva para as questões abertas. Resultado: 15,57% das clínicas odontológicas registradas no CRO/RS participaram da presente pesquisa. Foi possível verificar que a maioria dos materiais e técnicas aplicadas no tratamento endodôntico são os mesmos recomendados pela literatura científica. Conclusão: Os protocolos clínicos em endodontia seguidos pelos participantes do presente estudo apresentam embasamento clínico e científico na literatura.


Introduction: The evolution of new techniques, technologies, and materials significantly influences the clinical practice of endodontics, thus profoundly impacting the approach to root canal treatments. While information about various treatment methods practiced by dentists in other cities or countries is available, there is a notable gap in our understanding of endodontic practices in the Porto Alegre city. Objective: The objective of this study is to analyze the endodontic practices employed by dentists in dental clinics located in Porto Alegre city. We aimed to explore the techniques, materials, and equipment routinely utilized during endodontic treatments. Material and method: An online questionnaire using the Google Forms platform was administered, consisting of twenty-six open and closed questions to dentists performing endodontic treatments in dental clinics. The questionnaire was structured to assess the materials, techniques, and equipment employed during endodontic procedures. Data collected from the questionnaires were recorded in an Excel® spreadsheet. Data analysis encompassed both absolute and relative frequency for closed questions and also a descriptive approach for open-ended questions. Results: Participation was obtained from 15.57% of dental clinics registered with CRO/RS. The findings indicated that the majority of materials and techniques applied in endodontic treatment could be aligned with recommendations found in the scientific literature. Conclusion: The clinical protocols observed by the participants in endodontic practice demonstrated a solid foundation in both clinical and scientific literature.

ABSTRACT

Dens invaginatus is a rare developmental anomaly of the teeth that is caused by the infolding of enamel organs or the penetration of their proliferations into dental papillae before calcification has occurred. The presence of double dens invaginatus is extremely rare. This paper describes the use of cone beam computed tomography in the evaluation of a maxillary lateral incisor with double dens invaginatus and periapical periodontitis. The tooth was treated through microscopic root canal therapy. The tooth was free of clinical symptoms, and the periradicular lesion narrowed during the follow-up period of 1 year.

ABSTRACT

A ciência endodôntica possui um vasto conhecimento e com esse conhecimento os seus inúmeros questionamentos. Algumas teorias e conceitos mudam constantemente e trazem à tona contradições e divergências clínicas no âmbito biológico e prático. A patência foraminal é uma prática que permite que um instrumento de pequeno calibre ultrapasse o forame apical, com o intuito de limpar passivamente e prevenir o acúmulo de detritos e inibir a proliferação de microorganismos que podem causar infecções pós tratamento endodôntico. Essa prática gera discussão interna entre especialistas da área, mas as vantagens da técnica são inúmeras, principalmente no que diz respeito aos casos de polpa necrosada. Casos como esse não são solucionados com sucesso sem a utilização da patência apical. Por outro lado, existem contradições em relação aos casos em que a polpa está viva. Esse trabalho tem como objetivo analisar as vantagens e desvantagens da patência apical, bem como, se os benefícios excedem os possíveis danos que ela pode trazer(AU)


Endodontic science has vast knowledge and with this knowledge its countless questions. Some theories and concepts are constantly changing and bring to light clinical contradictions and divergences in the biological and practical scope. Foraminal patency is a practice that allows a small-caliber instrument to go beyond the apical foramen, in order to passively clean and prevent the accumulation of debris in the region and inhibit the proliferation of microorganisms that can cause infections after endodontic treatment. This practice generates internal discussion among specialists in the field, but the advantages of the technique are numerous, especially with regard to cases of necrotic pulp. Cases like this are not successfully resolved without the use of apical patency. On the other hand, there are contradictions regarding the cases where the pulp is alive. This work aims to analyze the advantages and disadvantages of apical patency, as well as whether the benefits exceed the harm it can bring(AU)

Background: A lot of videos on root canal treatment in children are available on YouTube™ but no quality checks are present. Addressing this is critical so that parents and dentists know why root canal in children is important including the explanation of procedural steps regarding treatment undertaken. Aim: The aim of this study is to analyze and grade YouTube™ videos on root canal treatment in children and assess whether it provides beneficial information for patients and dentists. Materials and Methods: Variables such as number of views, duration (minute), days since upload, number of comments, number of likes, number of dislikes, interaction index, and viewing rate were tabulated for selected 57 videos. Statistical Analysis: Selected videos were statistically analyzed using a 15-point scoring system and evaluated using SPSS software. Finally, the obtained data sets were classified as low-, moderate-, and high-content videos. Fisher's exact test was used to compare between proportions. Results: Based on the tabulated variables, most of the videos were low (n = 31) and moderate content (n = 24), which dentists mostly uploaded. There was no statistical significance between the video types and content score groups (P > 0.05). The most frequently mentioned issues were procedure (84.21%), obturating materials (75.44%), and etiology (63.16%). The least mentioned subjects were indications (24.56%) and importance (22.81%). Conclusion: From this study, YouTube™ videos on root canal treatment in children were of low and moderate quality. Hence, high-quality videos are essential for providing awareness among patients and dentists.

ABSTRACT

Este trabalho objetivou realizar duas revisões sistemáticas com as seguintes propostas: 1) Avaliar se a fototerapia com laser resultaria em menor dor pós-operatória (PP) em pacientes submetidos a reintervenção endodôntica; e 2) Avaliar se o uso da terapia fotodinâmica antimicrobiana (aPDT) seria eficaz na desinfecção de canais radiculares em casos de reintervenção endodôntica. As Revisões Sistemáticas foram registradas no PROSPERO (CRD42021243500 e CRD42021260013, respectivamente) e seguiram as diretrizes dos Itens de Relatório Preferenciais para Revisões Sistemáticas e Meta-análise (PRISMA). As buscas foram realizadas nas bases de dados eletrônicas PubMeb, Scopus, Web of Science, Embase, Web of Science, Clinical Trials e Cochrane Library e nos bancos de dados da literatura cinza. A qualidade metodológica e o risco de viés foram avaliados pela ferramenta Cochrane Risk of Bias para ensaios clínicos randomizados (RCT) e pelo qualificador NewcastleOttawa (NOS) para estudos não RCT (prospectivos). A análise da qualidade de evidência foi realizada com base na abordagem GRADE. A meta-análise foi realizada com o R software Meta package, utilizando um intervalo de confiança (IC) de 95%. Quanto aos resultados do Artigo 1: Cinco artigos foram incluídos para análise. Os estudos foram classificados como "baixo" risco de viés. Dos cinco estudos clínicos, quatro estudos mostraram uma diminuição significativa da PP após a reintervenção endodôntica nos grupos de fototerapia a laser quando comparados ao grupo controle, principalmente nos primeiros dias após a intervenção. A certeza de evidência foi classificada como baixa. Devido à alta heterogeneidade clínica entre os estudos, não foi possível realizar qualquer meta-análise. Apesar das limitações desta revisão sistemática, a fototerapia se mostrou uma alternativa promissora na redução e controle da PP na reintervenção endodôntica não cirúrgica. Nos resultados do Artigo 2, dez estudos atenderam aos critérios de elegibilidade e foram incluídos, sendo 8 utilizados na síntese quantitativa. A meta-análise mostrou que todos os dados dos estudos apresentaram diferença significativa antes e depois da terapia fotodinâmica antimicrobiana na redução da carga microbiana em infecções endondônticas secundárias (OR 0,15 [0,07; 0,32], p < 0,0001). No geral, os estudos apresentaram baixo risco de viés e a análise das evidências foi classificada como moderada. Sugere-se que a terapia fotodinâmica seja uma ferramenta benéfica e promissora, mostrando eficácia na redução da carga microbiana nos casos de reintervenção endodôntica. Em suma, a abordagem da utilização da fototerapia se demonstrou eficaz na diminuição da dor pós-operatória e na desinfecção dos canais radiculares, podendo ser uma terapia indicada nos casos de reintervenção endodôntica(AU)


This study aimed to carry out two systematic reviews with the following proposals: 1) Evaluate whether laser phototherapy would result in less postoperative pain (PP) in patients undergoing endodontic reintervention; and 2) Evaluate whether the use of antimicrobial photodynamic therapy (aPDT) would be effective in disinfection of root canals in cases of endodontic reintervention. Systematic Reviews were registered in PROSPERO (CRD42021243500 and CRD42021260013, respectively) and followed the guidelines of the Preferred Reporting Items for Systematic Reviews and Metaanalysis (PRISMA). Searches were performed in the electronic databases PubMeb, Scopus, Web of Science, Embase, Web of Science, Clinical Trials and Cochrane Library and in the gray literature databases. Methodological quality and risk of bias were assessed by the Cochrane Risk of Bias tool for randomized clinical trials (RCT) and by the Newcastle-Ottawa (NOS) qualifier for non-RCT (prospective) studies. The analysis of the quality of evidence was performed based on the GRADE approach. The meta-analysis was performed using the R Meta package software, using a 95% confidence interval (CI). As for the results of Article 1: Five articles were included for analysis. Studies were classified as "low" risk of bias. Of the five clinical studies, four studies showed a significant decrease in PP after endodontic reintervention in the laser phototherapy groups when compared to the control group, mainly in the first days after the intervention. The certainty of evidence was rated low. Due to the high clinical heterogeneity between studies, it was not possible to perform any meta-analysis. Despite the limitations of this systematic review, phototherapy proved to be a promising alternative for the reduction and control of PP in conventional endodontic reintervention. In the results of Article 2, ten studies met the eligibility criteria and were included, being 8 used in the quantitative synthesis. The meta-analysis showed that all data from the studies showed a significant difference before and after antimicrobial photodynamic therapy in reducing the microbial load in secondary endodontic infections (OR 0.15 [0.07; 0.32], p < 0.0001). Overall, the studies presented a low risk of bias and the analysis of evidence was rated as moderate. It is suggested that photodynamic therapy is a beneficial and promising tool, showing efficacy in reducing the microbial load in cases of endodontic reintervention. In summary, the approach to the use of phototherapy has been shown to be effective in reducing postoperative pain and disinfection of root canals, and may be an indicated therapy in cases of endodontic reintervention(AU)

ABSTRACT

Abstract This article reported two clinical cases in which the guided endodontics was used to perform the access to the root canals. The first case presents a 40-year-old female with a history of pain related to the left maxillary canine. After radiographic examination, the presence of severe calcification up to the apical third of the root canal, associated with a periapical radiolucency, was noted. In the second case, an 85-year-old male was referred to our service with pain upon palpation, at the right mandibular first molar. The radiographic images revealed the presence of endodontic treatment and a fiberglass post in the distal root canal, which was associated with extrusion of the filling material and a periapical lesion. The 3D-guides were planned based on cone beam computed tomography and intraoral digital scanning, which were aligned using a specific software. Therefore, implant drills could be guided up to the root canal length required for each case. In the first case, a surgical root canal was created and the patient was free of signs and symptoms after the treatment was completed. In the second case, it was observed that the fiber post was worn by the drill, allowing free access to the filling material. It was possible to perform the endodontic reintervention in a more predictable way and in less time. In both cases, the use of the guided endodontics allowed the preservation of a large part of the dental structure. The procedures were performed faster, without the occurrence of fractures and perforations.


Resumo Este artigo relatou dois casos clínicos em que a endodontia guiada foi utilizada para realizar o acesso aos canais radiculares. O primeiro caso apresenta uma mulher de 40 anos com história de dor relacionada ao canino superior esquerdo. Após exame radiográfico, notou-se a presença de calcificação acentuada até o terço apical do canal radicular, associada a radioluscência periapical. No segundo caso, um homem de 85 anos foi encaminhado ao nosso serviço com dor à palpação no primeiro molar inferior direito. As imagens radiográficas revelaram a presença de tratamento endodôntico e pino de fibra de vidro no canal radicular distal, que estava associado à extrusão do material obturador e lesão periapical. Os guias-3D foram planejados com base em tomografia computadorizada de feixe cônico e escaneamento intraoral digital, os quais foram alinhados por meio de um software específico. Desta forma, brocas de implante puderam ser guiadas até o comprimento necessário do canal radicular para cada caso. No primeiro caso, foi confeccionado um canal radicular cirúrgico e o paciente ficou sem sinais e sintomas após o término do tratamento. No segundo caso, observou-se que o pino de fibra foi desgastado pela broca, permitindo o livre acesso ao material obturador. Foi possível realizar a reintervenção endodôntica de forma mais previsível e em menos tempo. Em ambos os casos, o uso da endodôntica guiada permitiu a preservação de grande parte da estrutura dentária. Os procedimentos foram realizados com maior agilidade, sem a ocorrência de fraturas e perfurações.

El objetivo de este estudio es conocer la efectividad del gel de clorhexidina y pasta de hidróxido de calcio con clorhexidina como medicamento intracanal en la eliminación del Enterococcus faecalis (E. faecalis). Se realizó una revisión bibliográfica mediante una búsqueda exhaustiva en diferentes bases de datos como: PubMed, ScienceDirect, SciELO, Springer, Google Scholar. Se seleccionaron mediante los criterios de inclusión y exclusión 37 artículos publicados en el periodo 2003-2020, los cuales fueron enfocados especialmente en E. faecalis y medicación intracanal. Teniendo como resultados que la clorhexidina en mayor porcentaje resultó ser el medicamento intracanal más eficaz en un tiempo promedio de 7 días, seguida de la combinación de hidróxido de calcio más clorhexidina que actuó de igual manera eliminando el E. faecalis de los canales radiculares, mientras que el hidróxido de calcio no presentó actividad antimicrobiana frente al E. faecalis.


The aim of this study was to know the effectiveness of chlorhexidine gel and calcium hydroxide paste with chlorhexidine as intracanal medicaments in Enterococcus faecalis(E. faecalis) elimination. A bibliographic review was done through an exhaustive search in different databases such as: PubMed, ScienceDirect, Scielo, Springer, Google Scholar. Using inclusion and exclusion criteria, 37 articles published in the period 2003-2020 were selected, which were especially focused on E. faecalis and intracanal medication. The analysis showed that chlorhexidine in a higher percentage was the most effective intracanal medication in an average time of 7 days, followed by the combination of calcium hydroxide plus chlorhexidine that acted in the same way eliminating E. faecalisfrom the root canals, while calcium hydroxide did not show any antimicrobial activity against E. faecalis.

Abstract The objective of this critical review of literature is to discuss relevant clinical factors associated with root fractures (RF) visualized by using a new CBCT software. RF constitutes a common occurrence and a challenge in clinical practice, in which the diagnosis becomes essential for the definition of rapid and precise decision-making. The characterization of RF may involve different aspects, such as orientation of the fracture line (horizontal, vertical, oblique), root position of the fracture (cervical, middle, apical third), fracture's coronal-radicular position (coronary, coronal-radicular, radicular), continuity of the fracture (crack, incomplete fracture, complete), bone extension of the fracture (supraosseous, bone level, infraosseous fracture). Imaging examinations have been routinely used to aid in the RF diagnosis. Even with high-resolution cone-beam computed tomography (CBCT) scans, many doubts often remain about the diagnostic outcome. Many interferences in the analysis of image quality to determine the diagnosis are identified, such as the sharpness, the noise, light and dark artifacts, among others. The professional's knowledge is essential for identifying the different patterns of fracture lines and their repercussions on adjacent bone tissues, as well as for the analysis of artifacts that may hide or show similarities to fracture lines. Fractures lines and root fractures that may be associated with phantom conditions that mimic fractures should be carefully analyzed. CBCT is the exam indicated to identify a root fracture. It is also added to the success of the diagnosis that the professional has scientific knowledge, training and mastery of advanced CBCT software.


Resumo O objetivo desta revisão crítica da literatura é discutir os fatores clínicos relevantes associados às fraturas radiculares FR visualizados por meio de um novo software de CBCT. As fraturas radiculares constituem uma ocorrência comum e um desafio na prática clínica, em que o diagnóstico torna-se essencial para a definição de uma tomada de decisão rápida e precisa. A caracterização da FR pode envolver diversos aspectos, como orientação da linha de fratura (horizontal, vertical, oblíqua), posição radicular da fratura (cervical, médio, apical), posição coronoradicular da fratura (coronária, coronorradicular, radicular), continuidade da fratura (trinca, fratura incompleta, completa), extensão óssea da fratura (supraóssea, nível ósseo, fratura infraóssea). Os exames de imagens têm sido usados ​​rotineiramente para auxiliar no diagnóstico de RF. Mesmo com a tomografia computadorizada de feixe cônico (TCFC) de alta resolução, muitas vezes permanecem muitas dúvidas sobre o resultado do diagnóstico. São identificadas muitas interferências na análise da qualidade da imagem para determinar o diagnóstico, como a nitidez, o ruído, artefatos claros e escuros, dentre outros. O conhecimento do profissional é fundamental para identificar os diferentes padrões de linhas de fraturas e suas repercussões nos tecidos ósseos adjacentes, bem como para a análise de artefatos que podem ocultar ou apresentar semelhanças com as linhas de fraturas. As linhas de fraturas e fraturas radiculares que podem estar associadas às condições fantasmas que mimetizam fraturas devem ser analisadas cuidadosamente. A TCFC constitui o exame por imagem indicado para identificar uma fratura radicular. Acrescenta-se ao sucesso do diagnóstico o fato do profissional apresentar conhecimento científico, treinamento e domínio de softwares avançados de TCFC.

Resumen La miel y sus productos, a lo largo de la historia, han sido usados por diversas civilizaciones con fines terapéuticos debido a sus efectos curativos. En la actualidad existe un renovado interés en el uso de la apiterapia como coadyuvante en diversos tratamientos médicos, en donde destaca el propóleo por sus propiedades bactericidas, antiinflamatorias, anticancerígenas, entre otras. El objetivo de este artículo es describir un caso de un primer molar inferior izquierdo con diagnóstico de necrosis pulpar y periodontitis periapical asintomática que fue manejado con el uso de la mezcla de propóleo con hidróxido de calcio como medicación intraconducto entre citas, logrando observar un proceso de reparación ósea durante este lapso. Se concluye que el propóleo es una alternativa viable como coadyuvante del hidróxido de calcio en los casos que está indicada la medicación intraconducto.


Resumo O mel e seus produtos, ao longo da história, têm sido utilizados por várias civilizações para fins terapêuticos devido aos seus efeitos curativos. Atualmente, há um interesse renovado no uso da apiterapia como adjuvante em vários tratamentos médicos, onde a própolis se destaca por seu bactericida, antiinflamatório, anticâncer, entre outros. O objetivo deste artigo é descrever um caso de primeiro molar inferior esquerdo com diagnóstico de necrose pulpar e periodontite periapical assintomática que foi gerenciada com o uso da mistura de própolis com hidróxido de cálcio como medicamento intraconducto entre as consultas, conseguindo observar um processo de reparo ósseo durante esse período. Em conclusão, a própolis é uma alternativa viável como adjuvante ao hidróxido de cálcio nos casos indicados por medicação intraconducto.


Abstract Various civilizations have used honey and its products for therapeutic purposes throughout history because of their healing effects. There is a renewed interest in the use of apitherapy as an adjunct in various medical treatments. Propolis stands out for its bactericidal, anti-inflammatory, and anti-cancer properties, among others. This paper aims to describe the case of a lower left first molar diagnosed with pulp necrosis and asymptomatic periapical periodontitis that was managed with a mixture of propolis and calcium hydroxide as an intracanal medicament between appointments. A bone repair process was observed during this period. It is concluded that propolis is a viable alternative as a calcium hydroxide adjuvant when intracanal medication is indicated.

Objective@#To explore the key points of clinical diagnosis and treatment of three mesiobuccal root canals.@*Methods@# In the procedure of endodontic therapy for the upper left second molar with pulpitis, through root canal exploration under a dental microscope and cone beam CT (CBCT)-assisted imaging examination, it was confirmed that the left upper second molar contained 3 roots and 5 root canals, among which the third root canal existed in the mesiobuccal root. Combined with perfect root canal preparation, cleaning, disinfection, filling and minimally invasive inlay repair, the clinical symptoms were eliminated. The patients were followed up and the related literatures were reviewed. @* Results @# One- and two-year follow-ups showed that the tooth had no discomfort and could be used normally. X-ray revealed that the filling was complete, and the periapical tissue was normal. The results of the literature review showed that the incidence of three mesiobuccal root canals in maxillary second molars was 0.11%-4.2%. It is difficult to find additional root canals only by X-ray imaging. Dentists should further determine the number and anatomical shape of root canals by CBCT and operating microscopy. When there are three mesiobuccal root canals in maxillary molars, dentists should avoid overpreparation. Healthy tooth tissue is the key to good prognosis. @* Conclusion@#During root canal therapy, clinicians should consider the anatomical variation of the root canal, should always be alert to the existence of an extra root canal, and should use CBCT, operating microscopy, ultrasound and various auxiliary instruments to locate and treat the variant root canal.

@#Introduction: Assessment of root canal treatment (RCT) quality performed by undergraduate students is important in evaluating the teaching of endodontic. Hence, this study aims to evaluate the obturation quality of root canal treated teeth and the number of visits required by patients to complete RCT rendered by undergraduate students in School of Dental Sciences, Universiti Sains Malaysia (USM). Methods: All RCT cases performed by fourth year and final year undergraduate dental students in 2017/2018 were included in this study with a total of 258 teeth. Both digital and conventional periapical radiographs of all treated teeth were assessed. Evaluation of the obturation quality was classified as acceptable and unacceptable based on the length in relation to the radiographic apex and density of the obturation in relation to the presence of voids. The number of visits required to complete each RCT case by the students were also assessed. Results: Out of the total 258 teeth, 63.6% were maxillary and 36.4% were mandibular comprising 53.5% anteriors, 14.7% premolars and 31.8% molars. Overall acceptable obturation quality was 71.3%. There was significant association between obturation quality and type of teeth as well as location of teeth. Conclusion: Students in this institution performed significantly better obturation quality on anterior teeth than on premolars and molars as well as on maxillary teeth as compared to the mandibular teeth. Patients seemed to require more visits for RCT of molars than anteriors and premolars.

ABSTRACT

Objective: The aim of this clinical study was to evaluate the survival rate of a new pulpectomy protocol using 2% chlorhexidine digluconate gel and Feapex® paste for endodontic treatment in primary teeth. Material and Methods: A total of 105 pulpectomies were performed in anterior and posterior teeth of 48 infants (1-3 years old) with high caries experience with irreversible pulpitis or pulp necrosis. All treatments were performed by dental surgeons with no specialization in Pediatric Dentistry, under local anesthesia and rubberdam isolation. Manual files were used in conjunction with 2% chlorherixidine gel for root canal instrumentation, and Feapex® paste was used as a obturation material. The clinical and radiographic outcomes were collected by one trained independent evaluator with a follow-up period of 24months. Success was determined by the absence of pain, pathological mobility, pathologic bone rarefaction, pathological root resorption and soft tissue pathology around the affected tooth Survival of the endodontic treatment was evaluated by estimating survival rates through Kaplan-Meier curves. Cox Regression analysis with shared fragility were performed to evaluate the association between the independent variables to endodontic treatment failure (α=5%). Results: After 24 months, the treatment survival was 86% (SE=0.03). Root resorption at baseline was associated with a higher risk of failure (HR=2.81; CI=1.12-7.08; p=0.027). The survival rate of the endodontic treated teeth due to dental trauma was 100%, while teeth with dental caries had lower survival rate (85.05%; p<0.001*). Other variables analyzed included gender, age of the child, tooth position (incisor/molar), restoration type, obturation quality, and caries experience were not associated with treatment failure (p>0.05). Conclusion: The new protocol using 2% chlorhexidine digluconate and Feapex® presented a high survival rate and can be considered as a suitable protocol for pulpectomy in primary teeth.Trial Registration: REBEC (RBR-282s2f) (AU)


Objetivo: Avaliar a sobrevida de um novo protocolo para tratamento endodôntico (pulpectomia) em dentes decíduos utilizando gel de digluconato de clorexidina 2% e pasta Feapex®. Material e Métodos: Um total de 105 pulpectomias foram realizadas em dentes anteriores e posteriores diagnosticados com pulpite irreversível ou necrose pulpar em 48 crianças (1-3 anos de idade) com alta experiência de cárie. Todos os tratamentos foram realizados por cirurgiões-dentistas clínicos gerais, sob anestesia local e isolamento absoluto. Limas manuais foram utilizadas em conjunto com cloroherixidina 2% gel para instrumentação dos canais radiculares e pasta Feapex® foi utilizada como material de obturação. Os resultados clínicos e radiográficos foram coletados por um avaliador independente treinado com um período de acompanhamento de 24 meses. O sucesso foi determinado pela ausência de dor, mobilidade patológica, rarefação óssea patológica, reabsorção radicular patológica e ausência de fístula/abscesso ao redor do dente tratado. A sobrevida do tratamento endodôntico foi estimativa utilizando curvas de Kaplan-Meier. Análise de regressão de Cox com fragilidade compartilhada foi realizada para avaliar a associação entre as variáveis independentes com a falha do tratamento endodôntico (α = 5%). Resultados: Após 24 meses, a sobrevida do tratamento foi de 86% (EP = 0,03). A reabsorção radicular no início do estudo foi associada a um maior risco de falha (HR= 2,81; IC= 1,12-7,08; p= 0,027). Dentes tratados endodonticamente devido ao traumatismo dentário na dentição decídua apresentaram taxa de sobrevida de 100%, enquanto dentes com comprometimento pulpar devido à cárie dentária tiveram menor sobrevida (85,05%; p<0,001 *). Todas as outras variáveis analisadas como sexo, idade da criança, dente (incisivo/molar), tipo de restauração, qualidade da obturação e experiência de cárie não foram associadas à falha do tratamento (p> 0,05). Conclusão: O novo protocolo utilizando digluconato de clorexidina 2% e Feapex® apresentou alta sobrevida e pode ser considerado um protocolo adequado para pulpectomia em dentes decíduos.Registro do estudo clínico: REBEC (RBR-282s2f). Introdução: As obturações dos canais radiculares e os pinos intrarradiculares geralmente produzem artefatos de contraste branco e possíveis alterações volumétricas nas imagens de tomografia computadorizada de feixe cônico (TCFC). Esses artefatos, induzidos pela maior densidade dos materiais obturadores de canais radiculares, cimentos para colocação de coroa e pinos intracanais, podem potencialmente levar a interpretações incorretas, interferindo no diagnóstico de um volume adquirido de TCFC, principalmente na avaliação de potenciais fraturas e perfurações radiculares. Objetivos: Avaliar em seis casos clínicos os efeitos positivos e negativos dos artefatos de contraste branco no diagnóstico de fraturas radiculares, perfurações radiculares e cimentação de restaurações de porcelana. Métodos: O software e-Vol DX, que possui filtros específicos para redução de artefato de contraste (BAR, Blooming Artifact Reduction), foi usado para melhorar o valor diagnóstico dos volumes adquiridos de TCFC. Conclusões: O em- prego do software e-Vol DX para a reconstrução de imagens de TCFC melhorou a visualização das estruturas anatômicas e reduziu os artefatos. A visualização aprimorada de da- dos nas imagens pode auxiliar na identificação de detalhes essenciais que, em conjunto com os achados clínicos, são úteis para uma correta estruturação do diagnóstico (AU).


Introduction: Root canal fillings and intraradicular posts often create white contrast artifacts and possible volumetric changes on CBCT scans known as blooming artifacts. These alterations could lead to the incorrect interpretations reducing the diagnostic of an acquired CBCT volume, particularly when evaluating potential root fractures and root perforations. Methods and Results: In six clinical cases, the positive and negative effects of beam hardening artifacts on the diagnosis of root fractures, root perforations and porcelain restoration cementation were evaluated. These artifacts, induced by the higher density of root canal filling materials, cements for crown placement and intracanal posts, may potentially lead to inaccurate or false interpretations. A novel software, e-Vol DX, which has specific filters for blooming artifact reduction (BAR), was used to improve the diagnostic value of acquired CBCT volumes. Conclusions: The use of the e-Vol DX software package for the reconstruction of CBCT scans improved visualization of anatomical structures and reduced blooming artifacts. Improved data visualization may help reveal essential details that, in conjunction with clinical findings, are useful to achieve a correct diagnosis (AU).

Abstract This study evaluated the dimensions of intraradicular posts using a new cone beam CT (CBCT) software, and verified the potential of blooming artifact reduction. Sixty-three single-rooted human teeth were shaped, obturated, prepared for intracanal post placement and distributed into three groups: G1: anatomically customized prefabricated glass fiber posts; G2: low-fusion alloy posts; G3: gold alloy posts. After post fabrication and luting with RelyX U200®, specimens were sectioned axially at 9 mm from the root apex, and markings were made on the root surfaces (X-, Y- and Z-axes). The dimensions of the original posts (control group) were measured using a digital micrometer. CBCT scans of the teeth were obtained using a PreXion 3D Elite® scanner. Posts were measured on CBCT scans using DICOM files and the e-Vol DX software. A specific filter, Blooming Artefact Reduction (BAR), was developed to analyze intracanal posts. Statistical data were evaluated using the Van de Waerden nonparametric analysis of variance and, after that, normalized data were analyzed using the Tukey test. The level of significance was set at α = 5%. The measures of the anatomical prefabricated, low-fusion alloy and gold alloy intracanal posts obtained using the e-Vol DX CBCT software and a micrometer were not significantly different (p>0.05). The use of the BAR filter of the e-Vol DX software application did not induce any dimensional differences on CBCT scans of intracanal posts when compared with measurements made with a micrometer on original posts. The use of the BAR filter eliminated blooming artifacts.


Resumo Este estudo avaliou as dimensões de pinos intrarradiculares usando um novo software de tomografia computadorizada de feixe cônico (TCFC) e um micrômetro digital, e verificou o potencial da redução do artefato de contraste do branco. Sessenta e três dentes humanos unirradiculares foram modelados, obturados, preparados para colocação de pinos e distribuídos em três grupos: G1: pino de fibra de vidro pré-fabricado anatômico; G2: pino metálico de liga de baixa fusão; G3: pino metálico de liga de ouro. Após a confecção e cimentação dos pinos com RelyX U200®, os espécimes foram cortados no sentido axial a 9 mm do ápice radicular, sendo efetuadas marcações em suas superfícies radiculares (eixos X, Y e Z) para orientação das medidas e sincronizações das imagens. Foi utilizado um micrômetro digital para a mensuração das dimensões dos pinos originais (grupo controle) associado ao microscópio operatório. Posteriormente, as imagens em TCFC foram obtidas usando o PreXion 3D Elite®. Para a mensuração dos pinos nas imagens de TCFC utilizou-se o arquivo DICOM e a ferramenta de medida do software e-Vol DX, configurada para medidas milesimais, com o filtro BAR (Blooming Artifact Reduction). Os dados estatísticos foram avaliados com a análise de variância não-paramétrica de Van der Waerden, seguida pelo Teste de Tukey aplicado aos dados normalizados. O nível de significância foi de α = 5%. Os resultados mostraram que as medidas dos diâmetros dos pinos intrarradiculares (fibra de vidro anatômico, metálico liga de baixa fusão, e em liga de ouro) usando o software e-Vol DX e o micrômetro não mostram diferenças significativas entre si. O uso do filtro BAR do software e-Vol DX não induziu diferenças dimensionais nas imagens de TCFC dos pinos quando comparadas às medidas realizadas com o micrômetro sobre os pinos originais. O uso do filtro BAR eliminou artefatos de contraste do branco.

ABSTRACT

Root canal treatment is the most commonly performed procedure in dentistry in the field of endodontics. Prior to the procedure local anaesthesia is administered to ensure painless pulp extirpation and cleaning. There are various methods in which this is done by different practitioners, some prefer to administer a nerve block while a few only use an infiltration, and some use both. We wanted to assess the knowledge, awareness, and practice, regarding appropriate anaesthesia to be used while performing root canal therapy for mandibular molar, among practitioners and post graduate students in India. METHODSA total of 80 specialists volunteered to participate in the study. A simple online questionnaire with 10 relevant questions was framed and circulated to the participants. Responses that were received were verified and analysed using SPSS. RESULTS50 % of the respondents administered a classical inferior alveolar nerve block involving the IAN, lingual and long buccal nerve. 26 % of the respondents anesthetised the inferior alveolar nerve and lingual nerve and 17 % anesthetised the inferior alveolar nerve and long buccal nerve. CONCLUSIONSAnaesthesia of IAN alone should suffice during RCT of the mandibular molars. However, there is no adequate information and knowledge regarding the technique and the amount of local anaesthetic to be delivered for root canal therapy for mandibular molars thus inflicting further pain to the patient.

Abstract This study aimed to assess the physicochemical properties of a repair material in the Brazilian market, BioMTA, in comparison to other two materials currently in use (Biodentine and MTA Angelus). The initial setting time was evaluated using Gillmore needle. The pH was measured with a pH-meter after 24 h, 3, 7, 14 and 21 days. The radiopacity was determined using the equivalence in millimeters of aluminum (mm Al) from digitized occlusal radiographs. Solubility was determined after immersion in water for 7 days. The data were analyzed by one-way ANOVA and Tukey tests (a=0.05). The BioMTA initial setting time (5.2 min) was lower than the other materials (p<0.05). All materials showed an alkaline pH at 21 days. At 24 h, BioMTA was the most alkaline material (p<0.05); and at 3, 7, 14 and 21 days there was no difference between BioMTA and Biodentine (p>0.05), both being more alkaline than MTA Angelus (p<0.05). The radiopacity of BioMTA (4.2 mm Al) was significantly higher compared to Biodentine (p<0.05) and lower than MTA Angelus (p<0.05). The solubility of the materials was -4.2%, -1.6% and 4.1% for BioMTA, MTA Angelus and Biodentine, respectively, with a significant difference between them (p<0.05). Therefore, it can be concluded that BioMTA displayed a shorter setting time, an alkaline pH, a higher radiopacity, and a gain in mass.


Resumo Este estudo teve como objetivo avaliar as propriedades físico-químicas de um material reparador disponível no mercado brasileiro, BioMTA, em comparação com outros dois materiais atualmente em uso (Biodentine e MTA Angelus). O tempo de presa inicial foi avaliado usando a agulha de Gillmore. O pH foi medido com um pH-metro após 24 h, 3, 7, 14 e 21 dias. A radiopacidade foi determinada aplicando a equivalência em milímetros de alumínio (mm Al) de radiografias oclusais digitalizadas. A solubilidade foi determinada após imersão em água por 7 dias. Os dados foram analisados ​​por meio dos testes ANOVA de uma via e Tukey (a=0,05). O tempo de presa inicial do BioMTA (5,2 min) foi menor do que os outros materiais (p<0,05). Todos os materiais apresentaram pH alcalino aos 21 dias. Portanto, pode-se concluir que o BioMTA apresentou um menor tempo de presa, pH alcalino, uma alta radiopacidade e um ganho em massa.

ABSTRACT

Infection caused by microorganisms play a crucial role in the induction of inflammation of pulpal as well as the periapical tissues. Factors like improper disinfection or obturation of the root canal space contributes to the failure of the root canal treatment. The aim of the study was to modify the contents of MTA-based sealer with nanoparticles and check for its antibacterial efficacy against E. faecalis. Silver nanoparticles (10 nm) were incorporated in MTA based sealer at various concentrations to form the test product. The antibacterial efficacy of the modified sealer was tested by well diffusion test on E. faecalis. The zone of inhibition (mm) was checked for each test product. Analysis of the results showed significant diameters of zones of inhibition (mm) as compared to sealer without nanoparticles. The zone of inhibition increased with the increase in the concentration of silver nanoparticles. The silver nanoparticle incorporated sealer can be used in the clinical setup to prevent reinfection of the root canal system and ensure the success of the root canal treatment.

Introduction: Several determinants directly affect the Oral Health-related Quality of Life (OHRQoL) of root canal treatment (RCT) patients like knowledge, attitude, perceptions, age, gender, marital status, smoking and pre-exposure history of RCT. This study determined the sociodemographic determinants of OHRQoLamong patients underwent RCT.Methods: This cross-sectional study determiningthe OHRQoL of patients on RCTwas conducted among patients visiting various dental clinics. A self-administered and validated questionnaire comprising of four OHRQoL themes was used to collect the data.Statistical Package for Social Science (SPSS) ver. 22.0 was used to analyze the data.One-way ANOVA and independent t-test were used to determine the p-value.Results: For each theme of the OHRQoL research tool, its association was statistically significant with at least one demographic determinant ofthe RCT patients. A total of 26.3% of the participantswere males and 73.7% were females.Among the RCT patients, 38.5% were single whereas 61.5% were married. Conclusion: In conclusion, a moderately-good level of OHRQoL was observed among the patients on RCT.

Background: With the increase in life expectancy, there has been rise in cases of apical periodontitis. The present study was conducted to assess the prevalence of apical periodontitis and root canal treatment among the elderly and medically compromised patients in the Jammu population. Materials and Methods: The present study was conducted on 150 elderly patients with deep carious teeth of both genders. An equal number of healthy subjects was also recruited. Patients were divided into two groups: Group I was medically compromised patients with apical periodontitis and Group II was healthy subjects with apical periodontitis. Periapical status was scored based on the periapical index. Results: Group I had 95 and Group II had 60 patients with more than 1 periapical lesion. Group II had 110 and Group I had 48 patients with 1 periapical lesion with at least one obturated tooth. The difference was significant (P < 0.05). There was a nonsignificant difference between Groups I and II with more than 1 periapical lesions with at least one obturated tooth (P > 0.05). More than 1 periapical lesion was seen more in renal and respiratory patients (45), 1 periapical lesion with at least one obturated tooth was seen more one diabetic patients (22). The difference was non-significant (P > 0.05). Conclusion: The authors found that medically compromised patients have more periapical lesions with radiographic evidence. They have a lower frequency of obturated teeth than healthy subjects.
